WP3 Mortality Generic Protocol

Please note that whilst the document states 'Final Draft', as this is a generic protocol and each of the registries must make it registry-specific by amending it according to their own Ethics Committees requirements, there are instructions for the registries (in the form of track changes comments) to facilitate making it registry-specific. As such, they form part of the generic protocol.
